Georgia Pediatric GAPP Program
The Georgia Pediatric Program (GAPP) provides Medicaid-funded, homecare skilled nursing and personal care support to medically fragile children under 21. GAPP aims to keep children in their homes, allowing them to receive hospital-level care without frequent hospital visits. This program is designed to improve the quality of life for children with significant medical needs and their families by offering a familiar and comfortable environment.

Medicaid-funded:
GAPP is a Medicaid program, meaning eligible children can receive services at no cost to them or their families.
In-home care:
GAPP provides skilled nursing and personal care support directly in the child's home, allowing them to remain in a familiar and comfortable environment.
Skilled nursing:
This includes services like monitoring vital signs, medication administration, wound care, and assistance with medical equipment like ventilators or feeding tubes.
Personal care support:
This includes assistance with daily living activities like bathing, dressing, feeding, and mobility assistance.
Individualized care plans:
GAPP ensures each child receives a customized care plan developed by healthcare professionals to meet their specific needs.
Family-centered approach:
GAPP actively encourages family participation in the care process, fostering a supportive and nurturing environment.
Being eligible for GAPP, children must Be:
Be under 21 years of age.
Be Medicaid-eligible or eligible through the Katie Beckett Waiver.
Require skilled nursing or personal care due to a complex medical condition.
Have a physician document the medical necessity for GAPP services.
Weekly Updates
Our program consists of a weekly client update from our medical professional management team so that your child's service plan is revised continuously. We provide consistent updates on our clients' progress to their personal physician and primary caregiver (you) to ensure we are providing the best service and meeting each client's health needs.
Active learning
